ATC Consent Forms

Below is a list of the most common Consent Forms used in the ATC.

Cadets under 18 will need parents/carers to complete a form TG21 for every activity. If a cadet has a medical issue, additionally a TG23 form will need to be completed for every activity.

For flying and gliding activities, the TG21 (and TG23 if applicable) and the AV-MED1 forms are required.

Cadets who are over 18, as adults, may complete their own forms, but they are still required.

The reason why a new form is required for every activity is that emergency contact details can change, and medical issues can also come and go.
